Service Learning Hours Opportunity
Curie HS will be hosting a Life Source Blood Drive on Monday, November 27th.
You can earn 5 Service Learning Hours for your Blood Donation to Life Source!!! Please register for the upcoming blood drive by EITHER completing the Google Form during your lunch period on November 6,7,8 on the first floor in front of the bookstore. Or, complete the google form by November 8 using the attached link below.
You MUST Complete submit "PARENTAL CONSENT FORM" and "In School FUNCTION form" in order to participate. Even if you are 18, you must sign and return the form. The form can be picked up during the lunch periods on the dates listed above or you can print a copy of the attached form and bring it to the table by the lunchroom.
Please return your "PARENTAL CONSENT FORM" to Life source Blood Drive to volunteers/Life Source Blood Drive Representatives on Monday, November 13th during your LUNCH PERIOD at the same location by the bookstore.
No consent form & no google doc form = no school function sign out sheet = no service hours.
You will receive "On-Campus SCHOOL FUNCTION form" when you turn in your "PARENT CONSENT FORM."
Please bring completed "On-Campus SCHOOL FUNCTION form" to your assigned session on Monday, November 27th. This will be your ticket into Small GYM (Blue Gym).
