Chicago Public Schools is excited to welcome its students, their families, and members of the surrounding community to receive school-required and recommended health services, including, but not limited to immunizations. Insurance information is needed for some services (private or public-Medicaid/Medicare).

CONSENT FORM IS REQUIRED FOR MINORS. Consent form is attached below OR pick one up from the Nurses Office or the Main Entrance #1.
